Name / Email
Include a name, nickname or alias and an email address I can reach you with. Just in case I need to clarify something, or to let you know that I've approved your skin and/or when your skin request is live on the site.
URL
This is optional, I will mention/link you when/if your skin is approved and live under the credits and thanks section.
Series/Character/Theme/Etc
This is where you would let me know what series, character or whatever you want featured in a skin. For example: "Gohou Drug" or "Gensomaden Saiyuki: Genjo Sanzo"
Details & Etc
Include as much or as little information as you want. If you want to give me free reign, that's fine. If you have links to specific images or a specific idea or style you want replicated, explain or link them. You can attach images to your email, etc. Remember that I will only use images found in manga, artbooks and other paper-based publications. The reasoning is explained here.
Skins requested in progress. I try to create them as fast as I can, but it depends on my schedule. That and I tend to get pretty anal about layouts. Those marked as "non-request" are just themes I decided to create on my own.
